General information
Context
  Première Urgence Internationale (PUI) is a non-governmental, non-profit, non-political and non-religious international aid organization. Our teams are committed to supporting civilians’ victims of marginalization and exclusion, or hit by natural disasters, wars and economic collapses, by answering their fundamental needs. Our aim is to provide emergency relief to uprooted people in order to help them recover their dignity and regain self-sufficiency. The association leads on average 200 projects per year in the following sectors of intervention: food security, health, nutrition, construction and rehabilitation of infrastructures, water, sanitation, hygiene and economic recovery. PUI is providing assistance to around 7 million people in 21 countries – in Africa, Asia, Middle East, and Europe. PUI Libya mission started implementing operations in East (Benghazi and Alkufra) of Libya in 2017. The organization develop a lifesaving response that provides primary health care services to the most vulnerable population (internal displaced populations and their host communities, migrants, refugees and asylum seekers) combined with an integrated basic needs response to address the overall health situation of the PoC in detention centers and in urban settings.

General Context :

Libya, a mostly desert and oil-rich country with an ancient history, has more recently been known for the 42-year rule of the mercurial Colonel Muammar Gaddafi – and the chaos that has followed his departure (BBC, 2019). The country has splintered, and since 2014 has been divided into competing political and military factions based in Tripoli and the east. Among the key leaders are Prime Minister Fayez Sarraj, head of the internationally-recognized government in Tripoli, Khalifa Haftar; leader of the Libyan National Army, which controls much of eastern Libya; Aghila Saleh, speaker of the House of Representatives based in the eastern city of Tobruk; and Khaled Mishri, the elected head of the High State Council in Tripoli. 

Humanitarian consequences:

In 2020, the humanitarian situation in Libya is increasingly complex. The protracted nature of the conflict severely impacts on people’s wellbeing and livelihoods. Political stalemate has resulted in a governance vacuum, and coupled with widespread violence and insecurity, including direct attacks on public infrastructures, including health care infrastructures, and have disrupted the economy and public service delivery across the country. Furthermore, spikes in violence, such as the escalation of conflict in Tripoli since April 2019 and clashes in Murzuq in August 2019, have resulted in increased civilian casualties and further displacement (HNO, 2020). 

Across the country, over 301,000 Libyans remain displaced, including 128,000 people due to the Tripoli conflict, reversing the declining trend in displacement. As displacement has increased, so has the number of Libyans who are returning to their homes, around 447,000 people. Living conditions, including access to clean drinking water (36% of HHs report limited access to sufficient water), medical services (22% of PHFs are closed, 33% of PHFs lack of essential medicines), and safe housing (around 4,000 IDPs are in need of shelter) have all degraded due to the protracted situation in the country, particularly for women and children (HNO, 2020).

Despite the crisis, Libya remains an attractive destination for migrant workers due to an economy that relies on foreign labour, higher salaries and historical ties, as well as being a transitory route for people seeking opportunities or asylum in Europe. An estimated 655,000 refugees and migrants are in Libya, including 48,000 registered refugees and asylum seekers. However, refugees and migrants continue to be exposed to protection risks, human rights violations, exploitation and abuse (HNO, 2020).

PUI’s strategy/position in the country

The main programmatic objectives of PUI can be synthetized as follows:

  • Enhance protection environment and access to essential services for conflict-affected communities in Libya.
  • Provide life-saving assistance by ensuring a safe and dignified access to health services as well as an access to clean water, basic sanitation facilities and hygiene items for the affected families in Detention Centers. 
  • Improve the quality of the health services provided to the people through the national health facilities.

History of the mission and current programs

PUI Libya mission started implementing operations in East of Libya in 2017. The organization has developed a lifesaving response that provides primary health care services to the most vulnerable population (internal displaced populations and their host communities, migrants, refugees and asylum seekers) combined with an integrated basic needs response to address the overall health situation of the PoC in detention centres and in urban settings. As a result, after three years of experience, PUI is now positioning as a significant actor targeting all groups of population in various settings in all eastern Libya. Its constant and solid field presence enables the organization to develop strong relationships with key stakeholders (authorities, local community leaders and local and international NGOs), as well as acceptance in reference to the activities implementation. In 2020, 2 Mobile Health Teams are deployed in Kufra and 1 in Detention Centers in Eastern Libya. PUI is implementing an integrated approach, with Protection and MHPSS specialist embedded in each MHT. Additionally, PUI is working on the reinforcement of the health system through training and capacity building project for health professionals, and the rehabilitation of health facilities. To date, the mission is implementing 4 projects funding by AICS, EU, ECHO/ and UNHCR.

To note, due to the administrative context, the majority of the humanitarian intervention in Libya are being managed remotely from Tunis.

Overall Objective
The Pharmacy Technical Advisor is in charge of strengthening the pharmacy component of PUI health projects implemented in Libya. He/She is in charge of the capacity building of the national staff in charge of pharmacy management (2 pharmacist storekeepers and 1 mission pharmacist), developing standards and operating procedures (SOP) at all steps within the mission in order to ensure a better compliance of PUI pharmacy management guidelines and international standards.
Tasks and responsibilities
Technical support: He/she ensures the capacity building of the pharmacy team (Pharmacist and the Pharmacist Storekeepers) of the operational bases

Supply management: He/she is in charge of supervising the good management and documentation of all the supply chain for drugs and medical equipment in compliance with PUI procedures and donors’ guidelines. 

Stocks management: He/she is responsible for supervising the good management of the medical stocks at mission’s level. 

Data collection, treatment and analysis: He/she is responsible for the production of consumption data per project and their analysis and recommendation for efficient utilization of the medical stocks.

Procedures development: He/she is responsible for the development of procedures and SOP for the pharmacy at all levels adapted to the remote setting for the Libya mission

Coordination, reporting and dissemination of information: He/she is responsible for the use of all reporting tools, compile information and disseminate accordingly among concerned services, and the documents achieved in compliance with PUI procedures and donors’ guidelines. 

Specific objectives and related activities
  • TECHNICAL SUPPORT

He/she carries out an initial diagnosis of the pharmacy component of both bases and the central pharmacy.

He/she contributes to the identification of training needs of the pharmacy team.

He/she develops a capacity building plan for all the technical staffs involved in pharmacy management. The plan is scheduled over a defined period of time.

He/she provides training and technical support to the pharmacist and staff and storekeepers for Benghazi and Al-Kufrah on the following topics: stock management (including Sagastock), rational use, substitution, analysis of consumption, etc.

He/she is responsible for the setting of objectives for the pharmacy team 

He/she contributes the organization of the activities for the pharmacy component of the mission according to the context of each operational base, in close collaboration with the Field Coordinators & Medical Coordinator.

He/she defines priorities and objectives to be achieved by the Pharmacists and tracks their realisations through the appraisal process and final appraisal. 

He/she supports the pharmacist to provide effective support to the bases, contribute actively on drugs and medical equipment need assessment, and proper anticipation for efficient stock management.

  • SUPPLY CHAIN MANAGEMENT 
He/she makes the link with logistics and medical departments of Libya and HQ for the purchase of medicine and equipment, in accordance with PUI and donors procedures.

He/she is in charge of the needs assessment for the medical purchase requests issued for the ongoing projects

He/she places medical orders under the Med Co’s responsibility and monitor the international medical orders ongoing with the supply department in HQ.

  He/she actively contributes to the development of supply chains for specific products or kits.

  He/she contributes to the development of the country procurement framework for Libya regarding drugs, consumables & medical equipment. 

  In close collaboration with the logistics department and the HQ pharmacist, he/she actively participates in the audit process of Libyan suppliers.

  He/she participates, as required, in the technical evaluation of suppliers and supplies purchased locally in the respect of logistics and PUI quality assurance procedures (QUAMED audit recommendations, manufacturing site, etc.).

  He/she contributes in the validation of medical purchase files in collaboration with logistics.

  He/she anticipates, plans and supervises the drugs reception at mission’s level.

  1. STOCKS MANAGEMENT  

He/She ensures a complete traceability of stocks by project in compliance with donor procedures

He/She supervises the premises organization for medical stocks and provides technical recommendations for improvements and better compliance with international standards in terms of storage 

He/She ensures the understanding and application of procedures and tools used at the base level and health facilities supported

He/She is responsible for the supervision of the central pharmacy and medical stocks at the operational bases

He/She ensures the deployment and proper use of Sagastock and other IT stock management tools

He/She plans and ensures that regular inventories are carried out and shares the results for anticipating shortages or overstocks

He/She ensures each office, guest house, vehicle has a proper first aid kits in line with PUI guidelines and context-specific 

He/She supervises the stock management of particular pharmacies (vehicles, first aid kits, etc.). Inform the Med Co in case of irrational consumption.

He/She supervises the proper management of expired drugs and their disposal in line with the Health Authorities

He/She ensures pharmacovigilance including the immediate report of any problem with the quality of a drug in accordance with PUI pharmaceutical management procedures.

He/She validates orders sheets for the monthly supply of health units (MHTs, PHCC supported by PUI) and inform the Med Co in case of inconsistencies

He/She supervises the preparation, distribution planning, transport and delivery from central stock to Al-Kufra base. 

He/She supervises the preparation of donations / loans / exchanges at mission’s level.

He/She informs in advance and work with logistics department for the organization of transport of drugs.

  1. DATA COLLECTION, TREATMENT AND ANALYSIS 

He/she analyses the data to optimize the management of drugs and medical supplies (needs assessment, purchase, control, supply, cold chain, expiry dates, overstock/shortage etc.). 

He/she ensures regular and appropriate use of the management document (inventory records, delivery notes, computer tools etc.) by the Pharmacist Storekeepers for Benghazi and Al-Kufrah bases. 

He/she works closely with the medical teams to check, in relation to epidemiological data, the status of stocks and the consumptions and assess the appropriateness of prescriptions.

He/she is able to provide at any given time reliable information on stock, consumption, use of medicines and health products for each project. 

He/she contributes to the writing of donors’ reports specifically for information on pharmacy.

  • PROCEDURES DEVELOPMENT
He/she develops or updates pharmacy specific SOPs (for inventory, drugs reception, reception or performance of donation, etc.) taking into account the remote setting for Libya

He/she ensures that staff are trained on it as well as its proper use on the bases

He/she guarantees their capitalization at HQ level

 

  • COORDINATION, REPORTING AND DISSEMINATION OF INFORMATION 
He/she participates to any relevant internal and external meeting at base level and participates actively. 

He/she guarantees the technical coordination between both bases and from the bases to the coordination on all aspects related to pharmacy. 

He/she guarantees the proper dissemination of pharmacy tools in compliance with PUI procedures and donors’ guidelines. 

He/she is responsible to archive all the pharmacy documentation (all documents related to each single movement on each medical stock), according to the PUI and donors’ guidelines. 

He/she is responsible to send timely internal and external reports to the Med Co and the HQ technical referent (HQ pharmacist).

He/she shares a monthly backup (Sagastock) with the Medical Coordinator and the HQ referent.

The Pharmacy TA is the technical referent of the Pharmacist based in Benghazi (and indirectly the Pharmacist storekeepers) 

Focus on 3 priority activities related to the context of the mission 
    1. Building the capacity of the Pharmacist in order to ensure his full autonomy on his position
  • Develop specific SOP for the Libya mission in order to ensure the compliance with PUI pharmacy management guidelines
